[ARTICLES] The “Witch’s Chin” That Becomes Prominent When Smiling—Restoring Natural Expression Through Mentalis Muscle Repositioning

The so-called “witch’s chin,” a condition in which the chin appears excessively elongated, pointed, or pulled downward when smiling, is not merely a cosmetic concern. In many cases, it originates from abnormal tension and misalignment of the mentalis (chin) muscle. While it may be unnoticeable at rest, facial expressions such as smiling or speaking can cause the chin to sink inward or be drawn downward, a result of overactivity of the mentalis muscle and imbalance with surrounding facial muscles. Recently, mentalis muscle repositioning surgery has gained attention as an effective solution for this condition. Rather than altering bone structure, this procedure repositions and re-fixates the overactive chin muscle to its proper anatomical location, structurally correcting the distortion that appears during facial expressions. Because it can address the witch’s chin deformity without bone resection, patient satisfaction has been notably high. Dr. Jang Yong-jun of ARC Plastic Surgery e...

"If you are concerned with a double chin and protruding mouth"

Since our clinic performs facial contouring surgery, many people naturally ask about liposuction.

In particular, many people want to solve the double chin problem. Technically, the double chin is not just a matter of "fat."

For those with a small lower face, especially a more severe weak chin, the more the double chin comes along regardless of their body weight.

The weaker the chin is like the picture below, the chin tip is closer to the neck.

As there is not much space for skin tension, it looks more severe double chin even only putting the head down just a little bit.

Pre-Op

In the case of the male patient, he has a severely recessed chin as there is almost no curve under the lips.

For those like this patient, of course, their mandibular angle doesn't stand out because their bone itself is recessed backward and hidden inside the flesh.

Pre-Op

In the case of the female patient, she has a quite severe weak chin but the double chin doesn't seem to outstand because of the lengthy chin.
However, her mouth looks relatively protruded and the side profile balance doesn't seem to be good.

With the advancement genioplasty, there's a sense of distance between the tip of the chin and the neck, so the impression looks much more three-dimensional, the overall balance of the profile is improved, and the mouth is less protruding.

Pre-Op    VS    Post-Op

Of course, we shouldn't make a jutting chin as we aim for weak chin improvement.

However, it is relatively better for male patients to make the chin a little more advanced.

For those who have a double chin problem or have a protruding mouth, it is recommended to consult a specialist to find out the exact reason.
