[ARTICLES] The “Witch’s Chin” That Becomes Prominent When Smiling—Restoring Natural Expression Through Mentalis Muscle Repositioning

The so-called “witch’s chin,” a condition in which the chin appears excessively elongated, pointed, or pulled downward when smiling, is not merely a cosmetic concern. In many cases, it originates from abnormal tension and misalignment of the mentalis (chin) muscle. While it may be unnoticeable at rest, facial expressions such as smiling or speaking can cause the chin to sink inward or be drawn downward, a result of overactivity of the mentalis muscle and imbalance with surrounding facial muscles. Recently, mentalis muscle repositioning surgery has gained attention as an effective solution for this condition. Rather than altering bone structure, this procedure repositions and re-fixates the overactive chin muscle to its proper anatomical location, structurally correcting the distortion that appears during facial expressions. Because it can address the witch’s chin deformity without bone resection, patient satisfaction has been notably high. Dr. Jang Yong-jun of ARC Plastic Surgery e...

[ARTICLE] Witch’s Chin: More Than Just Sagging Skin — “Requires Precise Diagnosis of Chin Bone Structure

A protruding, drooping chin that resembles a so-called "witch’s chin" often results not simply from aging or sagging skin, but from a combination of underlying anatomical issues. These may include skeletal irregularities and soft tissue laxity. To effectively address this condition, surgical intervention tailored to the chin—rather than generic lifting procedures—is often required. Dr. Soonbeom Kwon, Chief Director of ARC Plastic Surgery, emphasizes, “Witch’s chin is not something that should be approached based on appearance alone. Precise analysis through CT imaging is crucial to evaluate the length, protrusion, symmetry, and soft tissue condition of the chin. Only with this level of diagnostic accuracy can we design a personalized surgical plan that truly improves the condition.” In many cases, lifting procedures alone are insufficient, especially when the chin structure is protruding or elongated downward. The surgical approach must be adjusted depending on whether the issue lies in the bone structure or in soft tissue laxity. Dr. Kwon warns, “If surgery is performed without a full understanding of the chin anatomy, it can lead to unsatisfactory outcomes or complications. Accurate diagnosis is essential for meaningful results.” He further states, “Chin contouring significantly impacts the central facial profile, which makes a detailed diagnosis and precise surgical technique imperative. Because surgical strategies must be tailored to each individual’s chin structure, it’s vital to work with a highly skilled surgeon who possesses extensive clinical experience and anatomical expertise.” Chin surgery and facial contouring must be conducted based on a deep understanding of individual skeletal structure and skin condition, supported by a broad range of clinical data and experience.
