[ARTICLES] Sagging After Facial Contouring Surgery, Improved with Zygoma Lifting

One of the relatively common side effects following facial contouring surgery is sagging in the cheekbone area. In particular, if bone fixation is incomplete or excessive bone reduction creates space between bone segments, the surrounding soft tissue of the zygoma may shift downward. This can result in overall midface sagging, causing the facial structure to appear collapsed or giving an aged impression. The most definitive solution known for such issues has traditionally been facelift surgery. However, many patients are reluctant to undergo this procedure due to the extensive incisions, long recovery period, and the risks of bleeding and swelling. As a result, temporal incision lifting has been gaining attention as an alternative, offering smaller incisions and faster recovery. Temporal incision lifting involves making a minimal incision within the hairline at the temple area, then elevating and securing the sagging midface tissues. Because it selectively approaches only the necessary areas without extensive dissection, the procedure carries less risk of bleeding, swelling, or pain, and allows for quicker recovery. Particularly when combined with an Endotine fixation device—a bioabsorbable material that stabilizes the lifted tissue for a period of time before safely dissolving in the body—the procedure provides secure results without leaving a foreign body sensation. Dr. Taekeun Yoon of ARC Plastic Surgery stated, “Sagging in the cheekbone area caused by volume loss and skin descent after contouring surgery is a common concern among patients. In many cases, temporal incision lifting can be a suitable solution to address this without the need for a full facelift.” He further emphasized, “Since surgical methods vary depending on skin elasticity, the degree and location of sagging, and individual bone structure, a tailored approach based on thorough consultation and anatomical analysis is essential.” Unlike simple skin tightening, temporal incision lifting re-establishes the structural support of the midface.
